重要通知「DM Care 糖讯通」迁移至 HA Go - DM Care

由于行政原因，目前的「DM Care 糖讯通」将迁移至「HA Go– DM Care」。

迁移完成后，现有的「DM Care 糖讯通」将停止运作，并且该应用程式将不再提供进一步的更新或支援。

HA Go–DM Care 应用程式包含有关糖尿病护理知识，为患者提供日常糖尿病自我管理所需的资讯。使用者可以将自己测量的血糖、血压读数，和个人饮食日记输入到应用程式中。

请在您的应用程式商店下载适用于 iOS 和 Android 装置的 HA Go。

下载 HA Go 后，请联络您的医护人员为您推送 “DM Care 糖讯通” 应用程式。

Important Notice: “DM Care 糖讯通” Migration to “HA Go – DM Care”

Thank you for using and supporting “DM Care 糖讯通” over the years.

Due to administrative reasons, the current “DM Care 糖讯通” will be migrating to “HA Go – DM Care”. Upon completion of the migration, the existing “DM Care 糖讯通” will cease to function and further updates or support for the app will no longer be available.

The HA Go – DM Care App encompasses useful tips on diabetes care providing patients with the support needed in their daily self-management of their diabetes. Users can record their self-measured blood glucose and blood pressure readings and enter records in a personal food diary in the app.

Please visit your app store to download HA Go which is available for both iOS and Android devices. Once you have downloaded HA Go, you may approach your HA health care provider to prescribe the DM Care App to you.
